Job DescriptionThis is a PRN/Part Time position for a Nurse Practitioner who will conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols.
ResponsibilitiesThe Nurse Practitioner will work collaboratively with physicians, case managers, social workers, family members, key caregivers and any appropriate ancillary medical personnel as necessary. They will also collaborate with Primary Care Physicians on patient education and provide follow-up.
Work EnvironmentNurse Practitioners will provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits. The salary for this position is $90 per completed assessment.
RequirementsTo be eligible for this position, applicants must have a Master's Degree or equivalent experience and licensure, current RN and NP licensure in the state of practice, board certification by the AANP, ANCC or AACCN, and a valid driver's license.
BenefitsPRN/Part Time providers receive non-benefited compensation except for 401(k) and limited voluntary benefits. Mileage reimbursement is also provided.
