Home Health Care Assistant

As a Home Health Aide with Great River Health, you will provide essential patient care under the guidance of a licensed nurse. Your role will involve assisting patients with daily living activities, serving as their 'Personal Care Champion' and providing care and support in the home setting or hospice house.
Key Responsibilities:
- Provide basic patient care under the direction of a licensed nurse.
- Assist patients with activities of daily living.
- Perform visits and/or shift work, working independently to provide personal care to individuals in their homes and facilities.
- Maintain accurate and timely documentation.
Requirements:
- Prefer Iowa Certified Nurses Aide.
- Maintain valid licensure/certification/registration if required.
- Current Drivers license and proof of insurance for staff performing visits.
