About the Role

We are seeking a compassionate and dedicated Caregiver Support Specialist to join our team at Phoenix Home Care and Hospice. As a Caregiver Support Specialist, you will play a vital role in providing exceptional care and support to individuals with intellectual disabilities, enabling them to live fulfilling lives.

Key Responsibilities
  • Assist with daily living activities, including meal preparation, personal care, and household chores.
  • Provide companionship and emotional support to individuals, promoting a sense of belonging and connection.
  • Accompany individuals into the community, fostering socialization and integration.
  • Collaborate with our team to develop and implement personalized care plans, ensuring the unique needs and goals of each individual are met.
  • Participate in ongoing training and professional development to enhance your skills and knowledge in caregiving and support.
Requirements
  • Be at least 18 years of age.
  • Have a high school diploma or equivalent.
  • Have a valid driver's license and reliable transportation.
  • Be able to pass a background check and drug test.
  • Be able to lift 50 pounds and perform physical tasks as required.
  • Have a minimum of six months of experience in a related field (personal or professional).
