Job SummaryWe're seeking a skilled Advanced Practice Clinician (Physician Assistant or Nurse Practitioner) to join our expanding house call practice. As a primary care provider, you'll perform Medicare Annual Wellness Visits and provide comprehensive care to patients in their homes, Board and Care facilities, and Assisted Living facilities.
Responsibilities- Perform primary care in the home setting, utilizing your expertise to deliver high-quality patient care.
- Drive to patient locations in your assigned geographic area, ensuring timely and efficient visits.
- Average 7-10 patient visits per day, providing personalized attention and care to each patient.
- Exhibit exceptional bedside manner, communicating complex medical information with simplicity and kindness.
- Provide health education to patients, empowering them to manage their health and prevent illness.
- Work with patients to prevent illness, promote health, and manage chronic health problems.
- Provide referrals to appropriate medical resources, ensuring seamless care coordination.
- Utilize our EMR system to document patient visits and interactions accurately and timely.
- Knowledge of HCC coding is essential for accurate documentation and billing.
- Active participation in an on-call rotation is required, ensuring 24/7 patient care.
- Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
- Current, unrestricted PA or NP license to practice in California, including DEA license.
- Minimum of 1 year of experience as an Advanced Practice Provider in primary care, hospice, palliative, or urgent care/outpatient setting.
- Previous experience in a home health, hospice, or other home visit setting is highly desired.
- Minimum of 1 year of previous experience utilizing an EMR system with a good working knowledge of HCC coding.
- Strong computer skills, including MS Office and Outlook.
- Great verbal and written communication skills.
- Current drivers' license, auto insurance, and reliable transportation.
- Spanish speakers are strongly preferred.
- Compassionate and caring attitude, with a desire to live our mission, vision, and values.
