Job OverviewWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a PRN/Part Time Home Risk Assessment professional. In this role, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
Responsibilities- Conduct comprehensive health assessments in various settings, including home visits, skilled facilities, and telehealth visits.
- Collaborate with physicians, case managers, social workers, family members, and other healthcare professionals to provide patient-centered care.
- Develop and implement individualized care plans to address patient needs and improve health outcomes.
- Provide education and support to patients and their families to promote healthy behaviors and disease management.
- Document patient information and outcomes in accordance with Matrix protocols and standards.
- Master's degree in Nursing or Physician Assistant studies, or equivalent experience and licensure.
- Current RN and NP or PA licensure in the state of practice, including prescription authority.
- Board certification in a Matrix-approved specialty.
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills.
