Job Title: Physical Therapist

Richmond Community Services, a leading non-profit provider of support services for individuals with developmental, intellectual, and behavioral challenges, is seeking a skilled Physical Therapist to join our team.

Job Summary:

We are looking for a highly motivated and experienced Physical Therapist to provide physical therapy services to our consumers in our ICF and group home locations. The successful candidate will work under the direction of the Coordinator of Rehabilitative Services to plan and implement prescribed physical therapy treatment programs.

Key Responsibilities:
  • Conduct annual screens, assessments, and daily documentation to ensure accurate and timely reporting of consumer progress.
  • Maintain active and current MD prescriptions as outlined by the NYS Department of Health.
  • Develop and implement treatment plans based on individual assessments, incorporating input from consumers, families, and direct support staff.
  • Administer treatment involving modalities, instruct and motivate individuals in exercise programs, and provide training to families and direct support staff.
  • Write specific, objective, and measurable documentation detailing consumer needs, potential for benefit, and progress achieved.
  • Consult with supervisor/manager to implement recommendations and provide training to staff.
  • Communicate effectively with staff regarding discharge planning and participate in conferences for individuals receiving physical therapy services.
  • Supervise PTA and therapists on limited permits, providing co-signatures on notes and performing clinical supervision.
Requirements:
  • Graduate of an accredited Physical Therapy school/program.
  • NYS License or eligibility for licensure to practice physical therapy.
  • Valid driver's license and reliable transportation for local travel.
  • 35-hour/week schedule, exempt from overtime pay.
