Music Therapist

The Care Team Home Health & Hospice is seeking a Music Therapist to join our team in Standish, MI. As a Music Therapist, you will provide music therapy services to patients in their homes or alternate community settings. This is a part-time position that requires a strong passion for music therapy and a commitment to delivering high-quality patient care.

• Assess and provide individualized music therapy consultations and services to patients and their families.
• Develop and implement music therapy plans of care that meet the unique needs of each patient.
• Use music to improve or maintain mental and physical function and quality of life for patients.
• Document all services provided to patients and their families in the clinical record.
• Collaborate with other members of the interdisciplinary team to ensure comprehensive patient care.

• Board Certified as a Music Therapist by the Certification Board for Music Therapy.
• Valid state driver's license and current automobile insurance.
• Current CPR certification.
• Ability to pass a criminal background check and MVR check.
