Music Therapist

We are seeking a skilled Music Therapist to join our team at The Care Team Home Health & Hospice. As a Music Therapist, you will play a vital role in providing therapeutic music services to patients in their homes or alternate community settings.

• Assess and provide individualized music therapy consultations and services to patients and their families.
• Develop and implement music therapy plans of care to improve or maintain mental and physical function and quality of life.
• Use creative therapeutic activities to enhance patient outcomes and provide emotional support.
• Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
• Document all services provided in the clinical record and maintain accurate records.

• Licensed or registered in the State of Michigan.
• Valid state driver's license and current automobile insurance.
• Current CPR certification and TB testing per agency policy.
• Board Certified as a Music Therapist by the Certification Board for Music Therapy preferred.
• Bachelor's Degree in Music with a concentration in Music Therapy or a Master's Degree in Music Therapy preferred.
