Job Overview:

ICONMA, L.L.C. is seeking a dedicated Charge Nurse RN to join our Long-Term Care team. In this role, you will be responsible for delivering comprehensive nursing care to residents while adhering to established care plans, facility protocols, and professional standards.

Your contributions will significantly impact the quality of care provided to residents, and you will oversee the work of Nursing Assistants to ensure compliance with care strategies.


Key Responsibilities:

- Conduct thorough nursing assessments to evaluate residents' health statuses and establish nursing diagnoses that guide care strategies.

- Formulate individualized care plans based on assessments and nursing diagnoses; implement nursing interventions and assess their effectiveness.

- Coordinate external physician visits to address residents' medical needs.
- Facilitate effective communication with physicians and family members to meet residents' needs.
- Provide education to residents, families, and staff to ensure clarity regarding the residents' care plans.
- Advocate for and uphold the philosophy of resident choices and rights.
- Administer and document routine and emergency medications and treatments as prescribed.
- Maintain a thorough understanding of medications and their potential side effects.
- Ensure accurate and timely documentation in clinical records to support continuity and quality of care.
- Implement safety and infection control measures to maintain a secure and hygienic environment for residents and staff.

- Collaborate with the interdisciplinary team to participate in Quality Management initiatives, ensuring that care standards are upheld within the facility.

- Serve as the primary contact on assigned shifts, effectively supervising resident assistants to ensure adherence to care plans.

- Demonstrate proficiency in IV therapy, including initiating IVs, administering initial medications, and managing central lines and catheters.
