Care Specialist

At THRIVE, we are seeking a dedicated and compassionate Care Specialist to join our team. As a Care Specialist, you will play a vital role in providing exceptional care and support to our residents, ensuring their physical, emotional, and social needs are met.
Responsibilities:- Verify individualized service plans and ensure appropriate care is provided to residents.
- Recognize and respond to residents' needs, promoting independence and dignity.
- Report changes in residents' conditions to supervisors and document observations.
- Assist with resident move-ins, transfers, and move-outs, ensuring a smooth transition.
- Perform routine care tasks, including bathing, dressing, and grooming, while maintaining confidentiality and respect for residents' rights.
- Monitor and report symptoms of potential skin breakdown and decubitus ulcers, intervening with appropriate skin care as instructed.
- Assist with the application of slings, elastic bandages, and other medical equipment as needed.
- Participate in orientation programs, on-going training, and educational classes to enhance skills and knowledge.
- Follow established safety precautions and report unsafe conditions in the facility.
- Document observed data on flow sheets, notes, charts, and other records in an informative and descriptive manner.
- Ability to work in a fast-paced environment, prioritizing tasks and managing time effectively.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with residents, families, and colleagues.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Physical stamina to perform tasks that require lifting, bending, and standing for prolonged periods.
