Job Summary

We are seeking an experienced Advanced Practice Provider to join our team at TruHealth LLC. As a key member of our healthcare team, you will be responsible for providing high-quality, patient-centered care to our long-term care residents.

Key Responsibilities
  • Assess patients' medical and mental health needs and develop personalized care plans to proactively manage their conditions and address barriers to care.
  • Ensure compliance with local, state, and federal regulations related to clinical services.
  • Prescribe medications and order lab work, diagnostic procedures, and consultations as needed.
  • Monitor patients' response to treatment and modify care plans as necessary.
  • Collaborate with RN case managers and other healthcare professionals to provide comprehensive care.
Requirements
  • FNP, AGNP, AHACNP, or PA license required.
  • Degree from an accredited APRN or PA program.
  • Experience in clinical nursing or rehab in geriatric populations.
  • Electronic Health Records experience.
  • Working knowledge of Microsoft applications, including Word, Outlook, and Excel.
Preferred Qualifications
  • Primary care experience in geriatric medicine.
  • 3+ years of experience in long-term care.
