Job Summary:

We're seeking a skilled RN Case Manager to join our team at Travel Nurse Across America. As a Case Management Travel Nurse, you'll work with a highly skilled team to advocate for patients, evaluate, plan, provide resources, and facilitate communication with family members. Your goal will be to coordinate comprehensive short and long-term care for patients, aiming to decrease hospital stay lengths and reduce readmission rates to generate cost-effective outcomes.

Key Responsibilities:
  • Evaluate and assess new patients to develop effective care plans.
  • Create and update comprehensive short and long-term patient care plans to ensure optimal outcomes.
  • Educate families and patients about available resources and healthcare options to promote informed decision-making.
  • Collaborate with other RNs, doctors, and social workers to provide seamless care coordination.
  • Properly document and submit medical records to maintain accurate patient information.
  • Communicate with insurance companies to resolve coverage issues and appeal decisions when necessary.
  • Analyze results, monitor variances, identify trends, and recommend actions to management to improve financial status.

Requirements:
  • BLS and ACLS certifications
  • 2 years of recent case management experience

Additional Information:
As a Case Management Travel Nurse, you should be able to stand and walk for long periods, carry up to 10 pounds of weight, and bend, lean, and stoop without difficulty. You'll work efficiently in a collaborative environment and possess good oral and written communication skills to interact effectively with patients, families, and coworkers.
