Job Summary

We are seeking a skilled Nurse Case Manager to join our team at Children's Healthcare of Atlanta. As a key member of our care coordination team, you will play a vital role in ensuring the seamless transition of patients from our facilities to their homes.

Key Responsibilities
  • Develop and implement comprehensive transition care plans in collaboration with the clinical team.
  • Cultivate effective relationships with members of the medical and nursing staff, social workers, and other stakeholders to drive the care coordination process.
  • Communicate with home health agencies, third-party payors, and community resources to coordinate discharge needs.
  • Facilitate care conferences, interdisciplinary rounds, and other meetings to ensure continuity of patient care.
  • Perform outpatient and clinic care coordination, monitoring patient care as they transition between services.
  • Refer cases identified as risk management, peer review, or quality issues to appropriate personnel.
Requirements
  • 3 years/4,500 hours of experience with a Bachelor of Science in Nursing (BSN) or a current Children's nurse with 5 years/7,500 hours of experience with an Associates degree in Nursing.
  • Graduation from an accredited school of nursing.
  • Moderate to expert computer skills.
  • Working knowledge of financial aspects of third-party payors and reimbursement.
  • Ability to successfully pass the Basic Windows Skill Assessment at 80% or higher rating within 30 days of employment.
