As a Part-time Nurse Practitioner, you'll play a vital role in our HouseCalls product, providing annual clinical assessments to patients and helping them navigate the healthcare system.
Key Responsibilities:
- Conduct in-home assessments, including evaluations of past medical history, medication reconciliation, and comprehensive physical exams
- Perform evidence-based practice screenings, including point-of-care testing as needed
- Identify diagnoses and develop care management plans to improve patient outcomes
- Communicate findings with patients' primary care physicians to ensure seamless care coordination
- Travel up to 85% of the time, with all travel expenses covered
Requirements:
- Active NP license or ability to obtain by start date
- Licensure must be unencumbered, with no open disciplinary actions
- Ability to work autonomously or under supervision, depending on jurisdiction
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care
- Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions
Preferred Qualifications:
- 1+ years of clinical experience in a related field
- Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
- Home health care or home visit experience
