At Interim HealthCare, we are seeking a dynamic Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:
- Participate in ongoing interdisciplinary assessment of the patient.
- Complete initial and ongoing comprehensive assessments of the patient's needs.
- Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
- Provide services that are ordered by the physician as indicated in the plan of care.
- Counsel and educate the patient and family in meeting healthcare related needs.
- Initiate appropriate preventative and rehabilitative nursing procedures.
- Refer to other services as needed.
- Monitor and evaluate outcomes of patient care.
- Communicate with appropriate staff regarding changes in the patient's needs.
- Supervise paraprofessional staff.
- Effectively communicate with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care.
- Communicate with all physicians involved in the plan of care and other health care practitioners related to the current plan of care.
- Participate in Quality Assessment and Performance Improvement (QAPI) program.
- Participate in in-services or training, as requested.
- Access personal health information (PHI) necessary to fulfill the above duties and responsibilities.

Requirements:
- Graduate of an accredited school of nursing and current RN License in SC or compact license.
- 2 years' experience as a nurse with 1 year of experience in home health.
- Proficient computer skills and workplace software knowledge.
- Work with people, function well under stress, use clinical judgment, be creative and accept challenges.
- Able to effectively deal with multiple changes based on patient needs and scheduling.
- Clinical proficiency in nursing, strong communication skills, and compassion.
- CPR Certification
- Valid driver's license and proof of insurance
