Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse to join our team at Bridge Senior Living. As a Licensed Practical Nurse, you will play a vital role in delivering high-quality care to our residents, working collaboratively with physicians and care associates to implement and coordinate care plans.

Responsibilities:
  • Assess, plan, implement, and evaluate resident care
  • Provide high-level care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing residents' overall condition and behavior
  • Assist the Director of Wellness in developing and updating resident care plans
  • Maintain ongoing communication with residents, families, and physicians regarding resident care
Requirements:
  • High school diploma or equivalent (GED)
  • LPN License in applicable state
  • First Aid & CPR Certified
  • At least 1 year of related work experience
