Note:

As mandated by the Centers for Medicare & Medicaid Services (CMS) in conjunction with the Centers for Disease Control and Prevention (CDC), COVID-19 vaccinations are required for all Medicare and Medicaid certified providers. Consequently, all employees must be fully vaccinated or possess a valid exemption.



SCHEDULE:
10 am - 6 pm, Monday - Friday with weekend rotation.

HOW YOU'LL MAKE A DIFFERENCE:
As a Hospice, General Inpatient Nurse-Case Manager, you will provide care with compassion at the forefront. Your role will involve delivering holistic, family-centered care while anticipating, preventing, and managing undesirable symptoms.

HOW YOU'LL WORK:


Your responsibilities will include providing end-of-life care and hospice services to patients within the Health System Inpatient Hospice setting, adhering to established standards of practice.

You will evaluate the necessity to adjust the level of care based on assessments and collaboration with the Attending Physician or Medical Director.


MAJOR AREAS OF RESPONSIBILITY:

Plan of Care:

Implement care strategies aimed at achieving comfortable and safe dying, self-determined life closure, and effective grieving for both patients and families.


Family Educator/Advocate:

Uphold the dignity of dying patients while respecting their unique spiritual and cultural beliefs, providing appropriate support at the time of death, including the management of medications and notification of funeral services, in accordance with agency policy.


Policies:

Complete all clinical documentation in line with agency protocols and Medicare/Federal guidelines.


Communication:

Offer referral sources detailed information regarding hospice programs and coordinate patient care for optimal transitions from hospital to hospice care settings.


Operations:

Participate in interdisciplinary team meetings to engage in comprehensive discussions, consultations, and evaluations of patient and family needs.



HARD & SOFT SKILLS:
Exhibit compassionate communication with a positive demeanor.
Demonstrate patience when interacting with patients, families, physicians, and colleagues.
Work independently with proven skills in patient care and education.
Maintain attention to detail, being observant and following directions meticulously.

REQUIREMENTS:


A Bachelor's Degree in Nursing is preferred, or a Graduate of a Diploma or Associate Degree program in Nursing within the state of employment.
A minimum of 3 years of inpatient clinical experience, preferably in hospice care.
Possess a valid driver's license and auto insurance.
Capable of meeting all physical demands of the role.
