Important Note:

In accordance with the regulations from the Centers for Medicare & Medicaid Services (CMS) and the Centers for Disease Control and Prevention (CDC), COVID-19 vaccinations are mandatory for all Medicare and Medicaid certified providers. Consequently, all employees must be fully vaccinated or possess a valid exemption.



SCHEDULE:
9am - 5pm, Monday - Friday with a weekend rotation once a month.

LOCATION:
Positioned at Royal Oak Hospital

YOUR IMPACT:
As a Hospice, General Inpatient Nurse-Case Manager, you will play a vital role in delivering care that is holistic and centered around the family. Your responsibilities will include anticipating, preventing, and managing undesirable symptoms or secondary conditions to enhance the quality of life for patients and their families.

YOUR ROLE:


You will be responsible for delivering end-of-life care and hospice services to patients within the Health System Inpatient Hospice environment, adhering to established standards of practice.

Your role will involve assessing the need for adjustments in care levels in collaboration with the Attending Physician or Medical Director.


KEY RESPONSIBILITIES:

Care Planning:

Implement care strategies that promote comfortable and safe dying, facilitate self-determined life closure, and support effective grieving for both patients and families.


Family Support and Advocacy:

Uphold the dignity of the dying patient while respecting their individual spiritual and cultural beliefs. Provide necessary support at the time of death, including managing medications and coordinating with funeral services as per agency policies.


Documentation:

Complete all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines.


Communication:

Offer referral sources detailed information about hospice programs and ensure the seamless coordination of patient care for optimal transitions from hospital to hospice settings.


Team Collaboration:

Participate in interdisciplinary team meetings to engage in comprehensive discussions regarding patient and family needs.



SKILLS REQUIRED:
Effective communicator with a compassionate demeanor.
Demonstrated patience when interacting with patients, families, physicians, and colleagues.
Ability to work independently with proven skills in patient care and education.
Meticulous attention to detail, keen observational skills, and the ability to follow directions are essential.

QUALIFICATIONS:


A Bachelor's Degree in Nursing is preferred, or a Graduate of a Diploma or Associate Degree program in Nursing within the state of employment.
A minimum of 3 years of inpatient clinical experience, preferably in hospice care.
A valid driver's license and auto insurance in your name as a driver.
Ability to meet all physical demands of the role.
