As a key member of our healthcare team, you will be responsible for ensuring the highest level of care for our residents. This includes monitoring and assessing their health needs, mentoring our care partners and medication technicians, and collaborating with physicians and families to develop effective care plans. You will also be responsible for maintaining accurate and detailed records of resident care, ensuring compliance with state regulations and community policies.

Key Responsibilities:
  • Assess and implement care plans for residents
  • Participate in resident and family care conferences
  • Communicate effectively with residents, families, and physicians
  • Monitor and document resident health conditions
  • Perform skilled nursing tasks as defined by community policy and procedure
  • Assist with medication management and ensure safe and effective dispensing
  • Review and update resident health records
  • Provide guidance and education to care team members
  • Collaborate with team members to ensure safe and effective delivery of resident care

Requirements:
  • Licensed Nurse in good standing with State Licensing Authority
  • Experience in training and team member development preferred
  • Ability to establish effective relationships with residents, families, and team members
  • Strong documentation skills and basic computer skills
  • Ability to develop systems for monitoring resident health conditions and ensuring efficient and safe delivery of resident care
  • Strong organizational skills and ability to re-prioritize daily tasks
  • Ability to represent the community in a positive and professional manner
  • Must meet all health-related requirements pertaining to state licensure
  • Must maintain current CPR/First Aid and Food Handlers credentials
  • Must maintain annual Continuing Education Credits
