Position Summary:


In collaboration with Health System, the Outpatient Diabetes Educator plays a pivotal role in educating patients with diabetes within outpatient environments.

This role involves working alongside other healthcare professionals to identify patients who would benefit from diabetes education and management.

The position encompasses multiple sites within the healthcare network.



Work Schedule:
Monday - Friday, Day Shift (approximately 8am - 5pm). No weekend shifts required.

Key Responsibilities:


Evaluate patients' comprehension of their condition, including chronic complications and care standards, through interviews and assessments, assisting them in establishing self-care objectives to enhance disease management.


Deliver both group and individual educational sessions; implement interventions within the scope of practice, such as medical nutrition therapy, self-monitoring of blood glucose, and medication management as necessary.

Collaborate with physicians and other healthcare providers to assess the diabetes management needs of patients and their families. Provide support to clinical staff involved in diabetes care.


Identify and facilitate ongoing education for outpatient clinicians who lead in diabetes management and education.

Serve as a resource for program staff, nursing, and ancillary departments.


Address a variety of challenges creatively and engage in problem-solving that requires analysis of diverse data.

May undertake Quality Coordinator responsibilities as needed.


Compliance and Quality Assurance:
Ensure adherence to the National Standards for Diabetes Self-Management Education and Support (DSMES) to maintain accreditation, including:
- Overseeing the planning, execution, evaluation, and quality enhancement of DSMES services.
- Ensuring that DSMES services are evidence-based and tailored to meet the needs of the population served.

Desired Skills and Attributes:
Self-sufficient team player capable of functioning in various environments.

Day shift availability.

Willingness to learn new systems and processes.

Exhibits sound judgment in selecting methods and techniques for problem resolution.

Qualifications:
Registered Dietitian

Certification as a Diabetes Care and Education Specialist (CDCES) is required within the first two years of employment.

Appropriate professional licensure/registration is mandatory.

Bachelor's degree required.
