Position Summary:
In collaboration with a Health System, the Outpatient Diabetes Educator plays a pivotal role in educating patients diagnosed with diabetes within outpatient environments. This role involves working alongside other healthcare professionals to identify suitable candidates for diabetes education and management programs.
Work Schedule: Monday to Friday, Day Shift (approximately 8 AM - 5 PM CT). No weekend shifts required.
Key Responsibilities:
- Evaluate patients' comprehension of their condition, including chronic complications and care standards, through interviews and assessments, aiding them in establishing self-care objectives to enhance disease management.
- Deliver both group and individual educational sessions; implement interventions within the scope of practice, including medical nutrition therapy, self-monitoring of blood glucose, and medication management as necessary.
- Collaborate with physicians and other healthcare providers to ascertain the diabetes management requirements of patients and their families.
- Identify, support, and provide ongoing education to outpatient clinicians who are instrumental in maintaining expertise in diabetes management and education.
- Serve as a resource for program staff, nursing, and ancillary department personnel.
- Address a variety of challenges creatively and engage in problem-solving that requires analyzing diverse data factors.
- May undertake Quality Coordinator responsibilities as needed, ensuring adherence to National Standards for Diabetes Self-Management Education and Support (DSMES) to maintain ADA recognition and AADE accreditation, including overseeing the planning, implementation, evaluation, and quality enhancement of DSMES services.
Required Skills and Attributes:
- Self-sufficient team player capable of functioning in various settings.
- Availability for day shifts.
- Eagerness to learn new systems and operational processes.
- Exhibits sound judgment in selecting methods and techniques for problem resolution.
Qualifications:
- Registered Dietitian.
- Certification as a Diabetes Care and Education Specialist (CDCES) is required within the first two years of employment.
- Appropriate professional licensure/registration is mandatory.
- Bachelor's degree in a relevant field.
