At Pinnacle Home Care Inc., we offer a competitive salary of $65,000 per year for our Home Care Professional - Patient Care Coordinator role in Florida.

Job Description:

As a Home Care Professional - Patient Care Coordinator with Pinnacle Home Care Inc., you will be responsible for delivering high-quality patient care in the comfort of their own homes. This position requires a strong commitment to community health education and a passion for making a difference in the lives of others.

Responsibilities:

  • Conduct initial assessments of patients and families to determine home care needs, including physical assessments and histories of current and previous illnesses.
  • Regularly re-evaluate patient nursing needs to ensure the highest level of care.
  • Develop and implement patient care plans, incorporating therapeutic, preventative, and rehabilitative nursing actions.
  • Administer medications and treatments as prescribed by physicians and provide direct patient care as defined by the State Nurse Practice Act.
  • Counsel patients and families on nursing and related needs, providing health care instructions as appropriate.
  • Identify discharge planning needs and implement strategies to ensure a smooth transition for patients.
  • Act as a case manager, coordinating patient care and ensuring that all necessary services are in place.

Qualifications:

  • Associate's or Bachelor's degree in Nursing, with one to two years of recent acute care experience in an institutional setting.
  • Must be licensed in the State as a Registered Nurse.
  • Current BLS certification.
  • Licensed driver with a reliable vehicle and insurance.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills.
  • Prolonged or considerable walking and standing, with the ability to lift, position, or transfer patients.
