Job Summary:

We are seeking a highly skilled and experienced Advanced Cardiovascular Nurse Specialist to join our team at Sutter Health. As a key member of our ambulatory specialty team, you will be responsible for providing high-quality patient care, education, and support to patients with complex cardiovascular conditions.

Responsibilities:

  • Coordinate and deliver comprehensive patient care, including assessment, planning, implementation, and evaluation of care plans.
  • Develop and implement individualized care plans that incorporate psychological, sociocultural, spiritual, economic, and lifestyle factors.
  • Collaborate with the care team to ensure seamless transitions of care and effective communication with patients, families, and healthcare providers.
  • Provide education and support to patients and families on cardiovascular health, disease management, and self-care strategies.
  • Stay up-to-date with the latest advancements in cardiovascular care and incorporate evidence-based practices into patient care.

Requirements:

  • Graduate from an accredited School of Nursing.
  • Current RN licensure in California.
  • Basic Life Support (BLS) certification.
  • 2 years of recent relevant experience in cardiovascular nursing.
  • Advanced knowledge of cardiovascular anatomy, physiology, and pathophysiology.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
