In this role, you will be responsible for coordinating and supporting the activities of our medical and psychiatric providers to deliver integrated care to individuals experiencing homelessness.

Key Responsibilities:

  • Coordinate and support the activities of the team's medical and psychiatric providers
  • Develop linkages with other healthcare providers and resolve insurance issues
  • Communicate with hospitals and emergency rooms during client acute treatment/hospitalization
  • Field work, including canvassing for clients in the field for psych/medical appointments
  • Meet monthly with team leadership and SMT providers to review and update medically vulnerable list

Requirements:

  • MSW required at hire, LMSW required within first 3 months of hire
  • Valid driver's license and a driving record that meets the agency's auto insurance guidelines
  • Flexibility in work hours; available to work evening, overnight and weekend shifts when required
