This position involves close collaboration with a multidisciplinary team to evaluate, strategize, and oversee the care of clients.

Key Responsibilities:
  • Design and oversee Individualized Family Support Plans
  • Deliver thorough case management and service coordination
  • Facilitate Family Support Team Meetings
  • Encourage health and wellness programs
  • Provide support to clients and their families
  • Connect families with community and social support resources

Required Qualifications:
