Job Summary:

Polk County, WI is seeking a skilled LPN or RN to join our team at Golden Age Manor. As a valued member of our healthcare team, you will provide high-quality nursing care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:

Provide professional nursing care to residents, including administering medications, performing treatments, and maintaining accurate records.
Collaborate with the Director of Nursing and Nurse Care Manager to develop and implement resident care plans, ensuring that each resident receives individualized attention and care.
Supervise and delegate tasks to non-professional nursing personnel, ensuring that all residents receive the highest level of care.
Participate in daily rounds, assessing residents, reviewing medications and clinical records, and making recommendations for improvement.
Maintain accurate and up-to-date records, including resident care plans, medication records, and incident reports.

Requirements:

Graduation from an accredited school of nursing.
Current LPN or RN license in the state of Wisconsin.
Ability to work collaboratively as part of a team, with excellent communication and interpersonal skills.
Strong problem-solving and critical thinking skills, with the ability to make sound judgments and decisions in a fast-paced environment.
Ability to maintain confidentiality and handle sensitive information with discretion.
