This role allows you to make a significant difference in the lives of patients and their families by delivering essential care in the comfort of their homes.


Responsibilities:
  • Deliver personal care services to patients.
  • Utilize safety principles and proper body mechanics while performing personal and supportive care techniques, including client ambulation, transfers, and assisting with mobility.
  • Plan and prepare nutritious meals, assisting with feeding as needed.
  • Perform homemaking and environmental services as assigned.
  • Observe, report, and document any changes in the patient's condition.
  • Follow specific care instructions prepared by a registered nurse.
  • Assist patients with prescribed exercises as directed by a nurse or therapist.
  • Conduct and record simple urine tests as required.
  • Complete other duties as assigned.

Qualifications:
  • High school diploma or equivalent.
  • Current state certification as a nurse assistant in accordance with state requirements.
  • Experience as a CNA within the last two years; if more than two years have passed, completion of a training and competency program is necessary.
  • Ability to successfully pass the competency evaluation of the care center.
  • Current CPR certification.
