About the Role:

This dynamic position will involve working closely with complex renal patients in various settings, including their homes, dialysis centers, and electronically as needed.

Key Responsibilities:

• Conduct thorough assessments of patients' medical, behavioral, pharmaceutical, and social needs to identify gaps in care and barriers to good health.
• Develop and implement personalized care plans that address identified needs, remove barriers to care, and improve patient health.
• C coordinate care by serving as an advocate and resource for patients, their families, and healthcare providers.
• Facilitate seamless transitions of care across various settings, minimizing avoidable readmissions.
• Educate patients on their renal condition and provide self-management support to promote better health outcomes.
• Monitor and evaluate patient progress, making adjustments to care plans as needed.

Requirements:

• 1+ years of nursing experience in case management or care management, preferably coordinating care across multiple settings.
• 2+ years of healthcare-related experience.
• Current, unrestricted compact Registered Nurse license.
• BLS certification from a licensed AHA or American Red Cross training facility or provider.
• Proven patient-centered approach and ability to act as a patient advocate.
• Excellent communication and analytical skills, with ability to work collaboratively with team members.
