Mass General Brigham is seeking a skilled Registered Nurse (RN) to fill the role of Home Health Nurse Navigator in our Cambridge, Massachusetts location.

Job Summary:

The Home Health Nurse Navigator is responsible for identifying eligible patients for the MGB Home Hospital program and conducting clinical and social eligibility and safety screening. This role requires strong communication and collaboration skills, as well as a deep understanding of the program and its goals.

Responsibilities:
  • Attend daily huddles with the Home Hospital team to gather clinical information and identify patients based on catchment area and plan of care.
  • Interact with the Emergency Department interdisciplinary team and ED Care Facilitators to assist patient identification and transfer to MGB Home Hospital.
  • Collaborate closely with the MGB Home Hospital Admitting Provider to notify patients who meet eligibility criteria and obtain consent for transfer.
  • Communicate and collaborate with Administrative Coordinator and Administrative Manager regarding patient transfers and census goals.
  • Utilize analytical and critical thinking in decision making and judgment to support Home Hospital transfers and census goals.
  • Serve as a role model, coach, and facilitator to promote thorough collaborative and respectful communication.
Requirements:
  • Current RN licensure in the state of Massachusetts.
  • Minimum 3 years emergency department, home hospital, or inpatient experience.
  • Varied experience preferred.
Working Conditions:

This role is based in a hospital environment, in an emergency department setting.
