Job Summary

We are seeking a highly skilled and compassionate Home Health RN Care Manager to join our team at NYU Winthrop. As a key member of our home health care team, you will be responsible for providing high-quality patient care and leading a team of licensed clinicians to ensure the best possible outcomes for our patients.

Key Responsibilities
  • Perform initial and ongoing evaluations of patient needs and develop outcome-oriented care plans that conform to physician orders.
  • Oversee the implementation of care plans and ensure that they incorporate teaching related to health maintenance, prevention, and safety.
  • Coordinate available resources to manage care plans and ensure stated outcomes are achieved.
  • Lead a team of licensed clinicians, including LPNs, LVNs, Home Health Aides, and Social Workers, to ensure an appropriate match between patient needs and the skills/abilities of the care team.
  • Collaborate with team members to ensure an effective care plan and evaluate progress toward goals.
Requirements
  • Licensed Registered Nurse with a minimum of two years of clinical home care experience.
  • Minimum of one year of experience in a clinical management role.
  • Current CPR certification.
  • Knowledge of state and federal home health agency regulations/Conditions of Participation and Compliance standards and regulations.
  • OASIS certified or willing to obtain certification within one year of hire.
  • Strong organization, communication, and human relations skills.
