LPN Job Description

Linden Grove Health Care Center is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN, you will provide high-quality care to our residents, working under the direction of a registered nurse.

Key Responsibilities:
  • Deliver a full range of nursing care to residents, including admitting, transferring, and discharging.
  • Conduct daily rounds to observe residents' physical and emotional status.
  • Administer medications and monitor resident care provided by unlicensed staff.
  • Maintain confidentiality of resident care information and promote a culture of safety.
Requirements:
  • Current licensure to practice as an LPN in the state.
  • Strong communication and organizational skills.
  • Basic knowledge of computer use or willingness to learn.
  • Experience in Skilled Nursing Facility (SNF) environment.
  • Nursing experience providing direct care to multiple residents.
