- Provide outcomes-driven psycho-educational counseling to address areas of need, treatment adherence, vocational/educational rehabilitation, and quality of life.
- Support patients in accessing community resources, including home health services, vocational rehabilitation, and other essential services.
- Work with the administrative support staff to maintain updated patient resource lists and develop training programs for staff on psychosocial topics.
- Masters in Social Work (MSW) degree required.
- 0-2 years of related experience.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Knowledge of social work theory and methods, as well as quality of life measurement instruments.
