Responsibilities:- Assess, plan, implement, and evaluate resident care
- Provide high-level care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing residents' overall condition and behavior
- Assist the Director of Wellness in developing and updating resident care plans
- Maintain ongoing communication with residents, families, and physicians regarding resident care
- High school diploma or equivalent (GED)
- LPN License in applicable state
- First Aid & CPR Certified
- At least 1 year of related work experience
