Responsibilities:
* Implement physicians' prescribed plans of care
* Conduct patient assessments and coordinate care
* Administer prescribed medication, treatments, and therapies
* Document care provided in our system of record
* Teach and train family members on health promotion and disease prevention
* Accept and write verbal orders from physicians as required
* Follow up with physicians, clients, family members, and staff on client problems and concerns
Requirements:
* Active, unencumbered LPN license in the state of practice
* Current CPR certification from American Red Cross or American Heart Association
* TB skin test (current within last 12 months)
* Six months prior hands-on nursing experience preferred
* Reliable transportation
