Aveanna Healthcare Private Duty Nurse LPN Job Description

At Aveanna Healthcare LLC, we are committed to providing exceptional home health services to medically fragile children and adults. As a Private Duty Nurse LPN, you will play a vital role in delivering high-quality care to our patients and their families.

Responsibilities:
  • Implement physicians' prescribed plans of care for our patients.
  • Conduct patient assessments and coordinate care with our interdisciplinary team.
  • Administer prescribed medication, treatments, and therapies as ordered by physicians.
  • Document care provided in our system of record in a timely and compliant manner.
  • Teach and train family members on health promotion and disease management.
  • Accept and write verbal orders from physicians as required and obtain co-signature of Case Manager on all verbal orders.
  • Follow up with physicians, clients, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition.

Qualifications:
  • Must have and maintain an active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice.
  • Current CPR certification from American Red Cross, American Heart Association, or BLS.
  • TB skin test (current within last 12 months).
  • Six months prior hands-on nursing experience preferred but not required.
  • Must have reliable transportation.
