Job Summary

We are seeking a dedicated and compassionate RN Case Manager to join our team. The successful candidate will be responsible for developing individualized care plans, conducting regular assessments, and communicating with patients and families to ensure seamless care coordination.

Key Responsibilities
  • Conduct initial assessments of patients and families to determine hospice care needs
  • Regularly re-evaluate patients' nursing needs and update care plans accordingly
  • Collaborate with the healthcare team, patients, and families to ensure continuous and well-coordinated care
  • Counsel patients and families on end-of-life needs and provide emotional support
  • Initiate preventative nursing procedures and communicate with physicians regarding patient needs
  • Supervise Hospice Aides and LPNs
Requirements
  • Current NJ RN License in good standing
  • Hospice experience preferred
  • Ability to travel to assigned areas
  • Reliable transportation with liability insurance
  • Strong communication and interpersonal skills
