Case Management Coordinator

About the Job:
Behavioral Health Services North Inc. is searching for a highly motivated and empathetic ESSHI Tenancy Support Worker to join our rapidly growing team.
The ideal candidate will have strong case management skills and experience working with individuals recovering from severe mental illness, substance use disorders, and/or homelessness.
Key Responsibilities:
- Manage a caseload of Northwood tenants, providing timely and accurate documentation of services.
- Collaborate with the treatment team to identify and coordinate resources for residents, including benefit counseling, mental health/substance use/primary care services, funding, transportation, and linkage to other services.
- Provide rehabilitative tenancy support services, both in-office and in-resident homes, as outlined in OMH Supportive Housing Guidelines, documenting all services accurately and timely.
- Possibly facilitate staff meetings in the Supervisor's absence.
- Submit receipts, check requests, and purchase order requests into Papersave.
- Track and submit data monthly for quality assurance purposes.
- Assist with onboarding/training new team members.
- Assist with screening/admitting and discharging program participants.
- Coordinate and facilitate regular on-site or off-site activities for residents to promote well-being/stability/sense of community.
- Clerical duties include data entering, filing, etc.
- Report facility/client maintenance concerns to the Northwoods Property Manager to ensure a safe living environment for program participants.
- Act as a liaison between the tenant and landlord to resolve housing issues quickly.
Requirements:
- Associate degree in human services-related field required, OR High School diploma and 3 years experience in the human services field required.
- Experience working with special needs populations, especially those recovering from severe mental illness, substance use disorders, and/or homelessness preferred.
- Excellent interpersonal, engagement, communication, time management, and organizational skills required.
- Ability to work independently and collaboratively.
- Good data entry skills and proficiency in Microsoft Office software.
- Valid NYS driver's license, acceptable to BHSN insurance carrier, for required local travel.
- Reliable transportation to travel locally.
- CPR certification within 120 days of employment.
Salary:$65,000 per annum
