Job Summary: The Patient Care Coordinator will work under the supervision of a licensed nurse to provide direct patient care and assist with daily living activities. The ideal candidate will have excellent communication skills, a compassionate nature, and a strong commitment to delivering high-quality patient care.

Responsibilities:
  • Provide direct patient care, including bathing, dressing, and feeding patients.
  • Assist patients with daily living activities, such as toileting and transferring.
  • Communicate effectively with patients, families, and healthcare team members.

Requirements:
  • Current LPN licensure.
  • Experience working in a long-term care setting preferred.
  • Excellent communication and interpersonal skills.

Compensation: $70,000 - $90,000 per year.
