ROLE DESCRIPTION
The Community Support Specialist will be tasked with delivering rehabilitation and support services to clients, facilitating community integration, behavioral assistance, and crisis intervention for minors, adolescents, adults, and families in accordance with therapeutic plans.
We are currently seeking enthusiastic and driven Community Support Specialists (PRP Direct Care Counselors) for our PRP program to engage with a diverse clientele.
Immediate openings are available for professionals with backgrounds in mental health or substance use, experience in psychiatric rehabilitation and case management, and strong verbal and written communication skills.
Key Responsibilities:
The primary function of the Community Support Specialist is to provide community integration, behavioral assistance, and crisis intervention services based on rehabilitation and therapeutic plans for minors, adolescents, adults, and families.
Job Duties:
- Conduct a minimum of 3 visits per month for minor clients and 6 visits per month for adult clients.
- Meet clients in their homes, office environments, or various community settings.
- Complete client progress documentation within 24 to 48 hours based on Individual Rehabilitation Plan objectives.
- Engage in onsite psycho-education, social skills, and wellness groups or activities with clients at least monthly.
- Collaborate with therapists on a monthly basis.
- Work closely with families and other professionals to coordinate services across different agencies and programs in the client's best interest.
- Provide services and support that meet clients' mental health needs as outlined in the Individual Rehabilitation Plan.
- Assist clients in accessing entitlement benefits and monitor benefit status monthly.
- Be available during mental health crises while on duty.
- Engage clients in building therapeutic relationships and developing daily life skills for community functioning.
- Implement interventions from rehabilitation and therapeutic behavioral service plans.
- Organize and lead therapeutic recreation, cultural enrichment, and skill-building activities.
- Contribute to individualized rehabilitation service plans.
- Ensure clients manage their medication effectively and perform other direct service duties to support the well-being of clients and their families.
- Help individuals enhance their independent living and social skills, promoting community integration and decision-making autonomy.
- Promote community resources for individual integration.
- Participate in organizing and administering summer programs.
Qualifications:
- Experience in a fast-paced behavioral health or substance use environment preferred.
- Proficient in developing and implementing Individual Rehabilitation Plans.
- Familiarity with state regulations and referral systems desirable.
- High level of proficiency in the field of behavioral health.
- Excellent communication and counseling skills.
- Ability to establish effective organizational and patient relationships.
- Knowledge of community resources.
- Strong organizational and care coordination abilities.
- Respectful and empathetic attitude towards individuals of diverse backgrounds and health conditions.
Experience Requirements:
- Minimum of two years direct care or related experience preferred, working with at-risk populations such as children, adolescents, adults, and families with emotional disabilities and mental health diagnoses.
- An Associate Degree is a plus.
- Valid driver's license and full insurance required.
