Key Responsibilities- Conduct thorough assessments of patients' physical and psychosocial needs, developing and implementing individualized care plans to ensure optimal outcomes.
- Collaborate with interdisciplinary teams to coordinate patient care, including medication management, wound care, and pain management.
- Provide education and support to patients and their families, empowering them to make informed decisions about their care.
- Document patient progress, including vital signs, medications, and treatment plans, ensuring accurate and timely communication with healthcare teams.
- Manage patient caseloads, prioritizing tasks and ensuring timely completion of responsibilities.
- Participate in ongoing education and training to stay current with best practices in hospice care and case management.
- Diploma, Associate, or Bachelor of Science in Nursing; BSN preferred.
- Current licensure as a Registered Nurse in the State of New Jersey; licensure in Pennsylvania if practicing in PA.
- Minimum one-year adult, pediatric, or medical/surgical clinical experience.
- Prior home care or hospice experience preferred.
