RN Case Manager

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.
Responsibilities- Conduct thorough assessments of patients' physical and psychosocial needs, developing and implementing individualized care plans to ensure optimal outcomes.
- Collaborate with healthcare professionals, patients, and families to coordinate care and address complex medical and social issues.
- Provide education and support to patients and families, empowering them to make informed decisions about their care.
- Manage patient caseloads, prioritizing tasks and ensuring timely completion of documentation and communication.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Current RN licensure in the State of New Jersey (or Pennsylvania if practicing in PA)
- Minimum one year of adult, pediatric, or medical/surgical clinical experience
- Strong assessment and documentation skills, with ability to work independently and as part of a team
- Excellent communication and interpersonal skills, with ability to empathize with patients and families
- Proficiency in electronic medical records and Microsoft applications
