LPN - Primary Care Nurse

We are seeking a skilled LPN to join our team at Lowell General Hospital. As a Primary Care Nurse, you will provide high-quality patient care and work collaboratively with our healthcare team to deliver exceptional patient experiences.
Key Responsibilities- Implement medical and nursing plans of care, medication administration, and procedures necessary for patient care under the direction of a Registered Nurse.
- Provide direct patient care within the primary nursing model to ensure continuity of care for all patients within your caseload.
- Perform technical aspects of patient care safely, according to established policies and procedures and Nurse Practice Act.
- Assess and reassess pain, utilizing appropriate pain management techniques and documenting pain management.
- Communicate all changes in patient condition immediately to the Registered Nurse.
- Bases all nursing interventions on pathophysiological and psychosocial rationale, delivering care in a fiscally responsible manner.
- Establish a positive relationship with patients and families, introducing yourself to patients and families as their nurse, providing teaching and counseling as needed.
- Review and educate discharged patients and families on ADLs, medications, treatments, and provide appropriate discharge instruction sheets.
- Document all nursing interventions and patient responses in the medical record for all assigned patients.
- Delegate responsibilities to others with an understanding of their roles, knowledge, and capabilities.
- Participate in performance improvement activities as requested, utilizing performance improvement principles to support and improve patient care.
- Massachusetts Licensed Practical Nurse (LPN) license.
- Experience dependent upon unit requirements and needs.
- Basic Life Support Certification.
- Cardiopulmonary Resuscitation (CPR) Instructor.
- Graduation from an accredited practical nursing program.
