We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Village Primary Care in North Chelmsford. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.
Key Responsibilities- Implement medical and nursing plans of care, nursing interventions, medication administration, and procedures necessary for patient care under the direction of a Registered Nurse.
- Provide direct patient care within the primary nursing model to ensure continuity of care for all patients within your caseload.
- Perform technical aspects of patient care (medications, treatments, etc.) safely, according to established policies and procedures and Nurse Practice Act.
- Assess and reassess pain, utilizing appropriate pain management techniques, and document pain management.
- Communicate all changes in patient condition immediately to the Registered Nurse.
- Bases all nursing interventions on pathophysiological and psychosocial rationale, delivering care in a fiscally responsible manner.
- Establish a positive relationship with patients and families, introducing yourself to patients and families as the patient's nurse, providing teaching and counseling according to their needs and ability to understand, and documenting teaching on the appropriate teaching tool.
- Reviews/educates discharged patients and families on ADLs, medications, treatments, and provides appropriate discharge instruction sheet(s).
- Documents all nursing interventions and patient response in the medical record for all assigned patients.
- Delegates responsibilities to others with an understanding of their roles, knowledge, and capabilities.
- Participates in performance improvement activities as requested, utilizing performance improvement principles to support and improve patient care.
- Holds self-accountable for professional practice and presents self in a calm and professional manner, keeping current with changing practices and interventions in patient care through lifelong learning.
- Maintains a collaborative work relationship with peers and colleagues in order to create a positive work environment.
- Reports pertinent observations and reactions regarding patients to the appropriate person (e.g., physicians, clinical manager, or charge nurse) and records those observations accurately and concisely.
