The LPN plays a crucial role in assessing patient and family needs, delivering compassionate care in alignment with physician directives and the comprehensive care plan established by the interdisciplinary team. Reporting to the Clinical Coordinator and Program Director, the LPN is integral in ensuring the comfort and well-being of patients and their families, adhering to our established policies and procedures.

We are looking for a motivated, empathetic, and skilled individual to fill the role of Hospice Licensed Practical Nurse (LPN) in the Nogales, Arizona region.

Qualifications:

  • Current licensure as an LPN/LVN in Arizona
  • Availability for some after-hours and weekend shifts
  • Strong verbal and written communication skills, along with organizational abilities
  • Valid driver's license with an insured vehicle in good condition
  • Current BLS certification
  • Proven nursing skills, with the ability to manage family emotional dynamics and respect diverse lifestyles
  • Possession or ability to obtain an Arizona class 1 fingerprint clearance card
  • Experience in Home Health is advantageous
  • Bilingual (English/Spanish) is highly preferred

Key Responsibilities:

  • Assessing patient and family needs while providing supportive care in accordance with physician orders and the interdisciplinary care plan
  • Collaborating with the assigned Nurse Case Manager to oversee nursing processes
  • Communicating with attending physicians, staff, and external agencies to coordinate optimal patient care
  • Maintaining consistent communication with the Nurse Case Manager, Program Director, and Medical Director
  • Gathering data on physical, psychological, social, and spiritual factors affecting patient and family health, and reporting findings to the assigned Registered Nurse Case Manager
