We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Dependable Health of Tucson. As a Hospice LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families in the Nogales, AZ area.
Key Responsibilities:- Assess patient/family needs and provide supportive care in accordance with physician orders and the interdisciplinary group plan of care.
- Collaborate with the Nurse Case Manager to coordinate optimal care and use of resources for patients and families.
- Communicate effectively with attending physicians, other staff members, and agencies as needed to ensure seamless care.
- Maintain regular communication with the Nurse Case Manager, Program Director, and Medical Director to ensure alignment with patient care goals.
- Current licensure as an LPN/LVN in the State of Arizona.
- Ability to work some after-hours and weekend shifts.
- Excellent verbal and written communication, and organizational skills.
- Valid driver's license and reliable vehicle.
- Current BLS certification.
- Ability to cope with family emotional stress and tolerance of individual lifestyles.
- Arizona class 1 fingerprint clearance card.
- Home Health experience a plus.
- Bilingual (English/Spanish) strongly preferred.
