Job Summary

OVP Health is seeking a highly skilled Case Manager to join our team. As a key member of our healthcare team, you will be responsible for coordinating and monitoring the assessment, treatment planning process, and follow-up for each resident. Your primary goal will be to serve as an advocate and liaison for and on behalf of residents' rights and benefits.

Key Responsibilities
  • Participate in client pre-placement planning, conducting orientations, and quarterly and annual assessments of individual client needs.
  • Ensure all participants complete the individual plan for contracted mental health services.
  • Coordinate and chair interdisciplinary treatment planning meetings and write individual plans of service.
  • Facilitate community resource availability based on client needs, ensuring maximum social integration.
  • Evaluate, facilitate, and monitor services and clients' progress in relation to established goals and objectives, documenting monthly in the medical record.
  • Provide monthly monitoring overviews and ensure all services have been delivered and properly accounted for before submitting billings to the fiscal office.
  • Evaluate and sign approval of the monthly home occupancy report and invoice.
  • Evaluate the monthly personal care service log.
  • Evaluate and approve the personal allowance fund record.
  • Facilitate client benefits, such as SSI, Medicaid, SSB, and Medicare.
  • Review and coordinate with the team and provider on decisions involving hospitalization, respite, and transfers/level-of-care changes, implementing plans of action and maintaining ongoing dialogue with intake regarding status changes.
  • Complete discharge notes in the medical record on planned discharges and comprehensive discharge summaries on planned/unplanned discharges within 14 days.
  • Provide crisis intervention for assigned caseloads.
  • Maintain assigned medical records as required by medical record procedures.
Qualifications
  • Bachelor's degree in social work or a human services field.
  • One year of work experience or a Master's degree in Social Work.
  • Two to three years of experience working in community mental health.
  • State social worker registration or equivalent license to be approved for eligibility as a qualified mental health professional.

  • Ability to calculate figures and amounts, such as percentages, mean, mode, and median.
  • Ability to draw and interpret bar graphs.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
Work Environment

This job operates in an outpatient facility environment. Working conditions described are representative of those that must be routinely met by and employee to successfully perform the essential duties of this position. This is a full-time position with scheduled hours of 8-4:30pm.
