Position Overview:

Role Responsibilities:
  • The Advanced Practice Nurse utilizes extensive education and clinical expertise to achieve optimal health outcomes for patients. This role involves addressing a variety of health issues that range in complexity from moderate to significant.
  • This is a full-time position, operating 40 hours per week, Monday through Friday, focusing on in-home primary care.

Key Duties:
  • Conduct health assessments and provide treatment for both acute and chronic conditions.
  • Prescribe medications, perform patient examinations, and diagnose illnesses.
  • Advise on strategic healthcare initiatives that impact patient care.
  • Exercise independent judgment and decision-making on complex healthcare issues.
  • Manage patient interactions with the goal of fostering strong relationships with patients and their families.
  • Utilize electronic medical records (EMR) for documentation and patient management.
  • Coordinate care management, including prescription refills and lab follow-ups.

Qualifications:
  • Bachelor's degree in nursing or related field.
  • A minimum of 2 years of clinical experience is required.
  • Possession of an active and unrestricted state nursing license, DEA license, and board certification.
  • Current driver's license and proof of auto insurance are mandatory.
  • Experience in senior patient care is essential.
  • Current Basic Life Support (BLS) certification through recognized organizations.
  • Strong technical skills and familiarity with healthcare technology.
  • Experience in leading projects is preferred.
  • This role involves direct patient interaction and may require participation in specific health screening programs.

Preferred Qualifications:
  • Master's degree in nursing.
  • Experience with house calls and in-home patient care.
  • Background in urgent care, primary care, or internal medicine.

Work Schedule:
This position operates on a standard day shift, with a weekly schedule of 5 days, Monday to Friday, with no weekend shifts required.
