Advanced Practice Nurse

Role Responsibilities:
- The Advanced Practice Nurse utilizes extensive education and clinical expertise to achieve optimal health outcomes for patients. This role involves addressing a variety of health issues that range in complexity from moderate to significant.
- This is a full-time position, operating 40 hours per week, Monday through Friday, focusing on in-home primary care.
Key Duties:
- Conduct health assessments and provide treatment for both acute and chronic conditions.
- Prescribe medications, perform patient examinations, and diagnose illnesses.
- Advise on strategic healthcare initiatives that impact patient care.
- Exercise independent judgment and decision-making on complex healthcare issues.
- Manage patient interactions with the goal of fostering strong relationships with patients and their families.
- Utilize electronic medical records (EMR) for documentation and patient management.
- Coordinate care management, including prescription refills and lab follow-ups.
Qualifications:
- Bachelor's degree in nursing or related field.
- A minimum of 2 years of clinical experience is required.
- Possession of an active and unrestricted state nursing license, DEA license, and board certification.
- Current driver's license and proof of auto insurance are mandatory.
- Experience in senior patient care is essential.
- Current Basic Life Support (BLS) certification through recognized organizations.
- Strong technical skills and familiarity with healthcare technology.
- Experience in leading projects is preferred.
- This role involves direct patient interaction and may require participation in specific health screening programs.

Work Schedule:
This position operates on a standard day shift, with a weekly schedule of 5 days, Monday to Friday, with no weekend shifts required.
