Job Summary:

The Registered Dietitian - Medical Nutrition Therapist provides Medical Nutrition Therapy and Diabetes Self-Management Education to L.Care Health Plan's members via telehealth or in-person.

This position uses science-based practice to assess, provide nutritional diagnosis, plan interventions based on member goals, monitor progress, and evaluate outcomes.

The role involves documenting and communicating interactions with pertinent partners and member's healthcare team. Additional responsibilities include identifying and collaborating with community-based organizations to develop/coordinate and/or review health education programs.

Key Responsibilities:

  • Provide Medical Nutrition Therapy upon receiving a treating provider referral/diet prescription
  • Develop curricula and conduct education for community groups and members on nutrition-related topics
  • Review and/or create health and nutrition education materials and articles for members, providers, and employees
  • Research, identify, and make recommendations regarding collaborations with external community-based organizations and/or individuals on nutrition or diabetes education-related matters

Requirements:

This position requires specialized expertise, knowledge, and training. The ideal candidate will have a strong understanding of nutrition and healthcare principles.
