Position Overview:
The Clinical Triage Nurse provides essential telephonic assessment and documentation services that necessitate clinical expertise and interpretation, serving as a vital link between patient care teams and primary care providers.
Key Responsibilities:
- Assess and prioritize telephone encounters, managing patient calls and messages effectively, while determining the need for escalation to the appropriate care team or clinician.
- Conduct urgent medical triage for patients, ensuring timely evaluations for efficient scheduling into clinician calendars.
- Analyze lab results, imaging reports, consultation notes, and clinical summaries, identifying those requiring clinician review.
- Facilitate effective communication among healthcare disciplines involved in patient care, providing pertinent information and recommendations to providers.
- Foster professional relationships with healthcare providers and facilities to support home care objectives.
- Enhance collaboration with nursing staff, management, and healthcare providers to promote effective working relationships.
- Assess individual patient needs for services, referrals, and education, delivering necessary information regarding care plans.
- Participate in required meetings and team huddles, seeking guidance and resources as needed for patient care.
- Offer mentorship and support to colleagues, acting as a resource for non-clinical staff.
- Coordinate team-based care in the absence of the Program Manager.
- Adhere to federal and local confidentiality regulations, including HIPAA, to ensure patient privacy.
- Follow organizational guidelines and policies for safeguarding patient demographic, clinical, and financial information.
- Perform additional job-related duties as assigned.
Qualifications:
- Must possess a valid Registered Nurse license in the State of Rhode Island or in accordance with the Nurse Licensure Compact agreement.
- Graduate from an accredited nursing program.
- Maintain a current Rhode Island RN license in good standing.
- Have 3-5 years of relevant nursing experience.
Skills and Abilities:
- Ability to collaborate effectively with colleagues to ensure high-quality outcomes.
- Proficient in working both independently and as part of a team, with strong self-direction capabilities.
- Skilled in utilizing technology, including electronic medical record (EMR) systems and Microsoft Office applications.
- Capable of thriving in a fast-paced and occasionally stressful environment.
- Strong critical thinking, problem-solving, and multi-tasking abilities.
- Excellent verbal and written communication skills.
- Committed to maintaining quality, safety, and infection control standards.
- Ability to perform standard clinical procedures in accordance with established protocols.
- Demonstrated professionalism and superior customer service skills.
Physical Requirements:
- Primarily remote work with daily virtual communications; occasional office presence may be required for meetings and training.
- Prolonged periods of sitting or standing while using a computer.
- Ability to lift up to 15 pounds and possess manual dexterity for operating office equipment.
- State-wide travel may be necessary for offsite meetings and training.
- Must hold a valid driver's license and maintain a clear driving record.
