As a Home Health LPN, you will work collaboratively with our interdisciplinary team to deliver compassionate and personalized care to patients with short-term illnesses or chronic conditions.

Responsibilities:

  • Provide nursing services to patients in their homes, adhering to established Corporation policies and procedures.
  • Work in an interdisciplinary relationship with other Home Health team members, case managers, and under the supervision of the Clinical Manager.
  • Assess patients' physical and emotional needs, developing and implementing individualized care plans to meet their unique requirements.
  • Administer medications, perform wound care, and provide other nursing interventions as needed.
  • Collaborate with patients, families, and caregivers to ensure seamless care transitions and optimal patient outcomes.

Qualifications:

  • Graduate of an accredited school of nursing.
  • Current PA LPN license.
  • BLS Certification.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
